Clinical AI Governance Framework
A practical governance guide for approving, monitoring, and documenting clinical AI use without creating unnecessary bureaucracy.
Governance is what turns AI adoption from scattered experimentation into an accountable clinical program. In many practices, tools arrive through side doors: a physician signs up for an ambient scribe, a manager tests an intake chatbot, or a billing lead uses generative AI to draft appeals. Each individual use case can feel small, but together they create real regulatory, operational, and patient-safety exposure. This framework gives practices a lightweight structure for deciding which tools can be used, by whom, for what purpose, and under what review standards.
The framework begins with ownership. Someone has to maintain the approved tool list, the review criteria, and the re-evaluation calendar. In a solo practice that may be the physician owner; in a larger group it may be compliance, IT, or an operations leader with authority to say no. The intake step should require a clear use case, the data touched, the vendor name, the decision maker, and the expected benefit. That information sounds simple, but it prevents the common situation where staff are using an AI tool and leadership cannot explain what the tool actually does.
The next layer is policy. A strong governance framework distinguishes approved uses from prohibited uses, defines the level of human review required, and spells out how AI assistance should be documented. It should also address incident response. If a scribe inserts information that was never discussed, or a messaging tool drafts unsafe advice, the practice needs a predictable way to escalate, investigate, and temporarily pause the workflow. Governance should not live in a binder that no one reads. It should show up in onboarding, in audit conversations, and in quarterly reviews of whether the tool is still worth the risk.
The final layer is measurement. Practices should track not only adoption and cost, but also correction burden, near misses, user complaints, and any changes in documentation quality or turnaround time. A framework becomes credible when it helps leaders retire weak tools as confidently as they approve promising ones. That is especially important in clinical AI, where vendor claims evolve faster than evidence. Good governance does not slow innovation for its own sake. It creates enough structure that physicians can adopt high-value tools without normalizing preventable risk.
